Raukawa Charitable Trust is seeking dedicated and culturally grounded Whānau Ora Chronic Care Navigator (Kaiārahi) to join our team. This pivotal role specifically supports kaumātua to achieve their aspirations through holistic, whānau-centred planning and access to essential services.
This role is based in Putaruru and supports whanau within the Raukawa takiwā.

As a Kaiārahi, you will:
This role is part of a new hybrid service model, combining collective approaches to meet the diverse needs of whānau across our rohe; and will support whanau within our Chronic Care team.
Other skills and abilities required for this role includes the ability to support and assess the needs of whānau, the ability to write aspirational plans, and then carry out regular evaluations and reviews of those plans.
