Our service provides home-based supports to a wide range of people, including the elderly and those with chronic illness or a disability. Our team of Care Managers provide clinical expertise and support for our Clients and Support Workers.  

You will enjoy the flexibility and autonomy of this role, managing a caseload of community clients, visiting them in their home and supporting them to live safely and independently.

You will be well supported in this role, as we are a fun and supportive team of Care Managers, Care Placement Coordinators, Support Workers and other staff.

This position will be required to:

  • Provide comprehensive assessment for clients & collaboratively develop individual service plans
  • Assist with learning, development & training of the Support Workers
  • Exhibit excellence in customer service within a safety & quality focused framework & in a highly flexible homecare environment
  • Undertake administration tasks related to the role
  • Market & promote the business as required

The successful candidate will have:

  • A current practicing certificate along with a minimum of 2 – 3 years of recent experience in a hospital setting; aged care or disability sector
  • Experience or knowledge of the Home Healthcare setting
  • Excellent time management, planning and prioritising skills with the ability to maintain focus whilst under pressure
  • Excellent written and oral skills with the ability to communicate well with people from all ethnicities and walks of life
