Miranda Smith Homecare was established in Auckland in 1999 and is now based around the North Island and Canterbury.   We specialise in aged care in the community.   All our clients are vulnerable in some way, either physically or cognitively and we also provide palliative and end of life care.  

We have an exciting opportunity in the Hawkes Bay region for a part-time Registered Nurse to join our team as a Clinical Coordinator.    This is a varied role, working both in the office and the community, visiting clients and overseeing our wonderful team of carers. We are committed to providing compassionate care to our clients in the comfort of their own homes.  You will be joining a supportive team of healthcare professionals who value team work and excellence.  This would suit a Registered Nurse who is looking for a change in pace in their career, a role which will allow you to use your expertise to guide and educate our cares and clients.

While providing wrap-around case management and clinical oversight to private clients in the community, you will also be supporting the Hawkes Bay Branch Manager with rostering and training to ensure our carers are working within the scope of their scope. 

The role will include:

Assistance with Hospital discharge & setting up new clients * Assessing clients requirements of care * Writing a Service Delivery Plan * Writing a Medication Plan for carers to follow * Helping the team choose a suitable carer/s * Training carers for specific roles * Medication management and oversight * Ongoing support for clients and carers * Working alongside Hospice when required * One on One relationships with client, family and care team.

Our core office hours are 9am-4pm, Monday to Friday. This role is required to work 3 days per week, ideally Monday, Wednesday and Friday, but we can offer flexibility for the right candidate.  The role can start immediately.
