Royal District Nursing Services NZ (RDNS NZ) is a not-for-profit organization providing Home and Community-based Nursing Services across Northland, Greater Auckland, and Southland/Otago/Canterbury regions.  At RDNS NZ, we believe people receive the best care in the place they feel most comfortable – their own home. We're looking for an experienced Registered Nurse to join our Auckland team as a Clinical Coordinator, supporting people to maintain their independence, wellbeing and quality of life within their communities. 

About the Role

As a Clinical Coordinator, you'll manage your own caseload across Auckland, working closely with clients, families, support workers and healthcare professionals to deliver high-quality community-based care.

Your day may include:

  • Completing comprehensive client assessments, including InterRAI assessments
  • Developing person-centred care and support plans
  • Coordinating services to meet changing client needs
  • Providing clinical guidance and oversight to support workers
  • Working collaboratively with GPs, Allied Health providers, ACC and community services
  • Monitoring outcomes and identifying opportunities to improve client wellbeing
  • Building trusted relationships with clients and their whānau

What You'll Bring

We're looking for someone who combines strong clinical skills with a genuine passion for community-based care.

You'll have:

  • Current NZ Nursing Council registration and Annual Practising Certificate (APC)
  • Experience working in community, primary care, rehabilitation, aged care or similar settings
  • Excellent clinical assessment and care planning skills
  • Strong communication and relationship-building abilities
  • Confidence working independently and managing competing priorities
  • A full New Zealand Driver Licence
  • Eligibility to work in New Zealand
