RN – Community Nursing & Care Coordination
Royal District Nursing Services NZ (RDNS NZ) is a not-for-profit organization providing Home and Community-based Nursing Services across Northland, Greater Auckland, and Southland/Otago/Canterbury regions. At RDNS NZ, we believe people receive the best care in the place they feel most comfortable – their own home. We're looking for an experienced Registered Nurse to join our Auckland team as a Clinical Coordinator, supporting people to maintain their independence, wellbeing and quality of life within their communities.
About the Role
As a Clinical Coordinator, you'll manage your own caseload across Auckland, working closely with clients, families, support workers and healthcare professionals to deliver high-quality community-based care.
Your day may include:
- Completing comprehensive client assessments, including InterRAI assessments
- Developing person-centred care and support plans
- Coordinating services to meet changing client needs
- Providing clinical guidance and oversight to support workers
- Working collaboratively with GPs, Allied Health providers, ACC and community services
- Monitoring outcomes and identifying opportunities to improve client wellbeing
- Building trusted relationships with clients and their whānau

What You'll Bring
We're looking for someone who combines strong clinical skills with a genuine passion for community-based care.
You'll have:
- Current NZ Nursing Council registration and Annual Practising Certificate (APC)
- Experience working in community, primary care, rehabilitation, aged care or similar settings
- Excellent clinical assessment and care planning skills
- Strong communication and relationship-building abilities
- Confidence working independently and managing competing priorities
- A full New Zealand Driver Licence
- Eligibility to work in New Zealand
