We are recruiting for a Nurse Practitioner to join our whānau-focused Hauora Heretaunga Medical practice.
At Hauora Heretaunga, our Cornerstone Accredited teaching practice, focuses on delivering exceptional, evidence-based primary care services to our diverse patient population of over 6,000 whānau. Our model of care is similar to the Health Care Homes framework. We prioritise improving equity and health outcomes of our local community, with the majority of our clients being Māori and Pasifika.

Role Overview:
As a Nurse Practitioner you will be empowered to make decisions and recommendations to facilitate high-quality care and coordination for whānau with complex health needs. Your work will involve collaborating across practitioners and health and social services to ensure seamless transitions of care, ultimately improving health outcomes.

Ideally 32 hours per week [0.8 FTE]. Our clinic is open 8.30am to 5.00pm Monday to Friday.
No weekends or shift rosters.
