Continuing Care Practitioner - Community Hub

We are now seeking a full time permanent Continuing Care Practitioner – Community Hub who will support clients at our community hub based in New Lynn. We are looking for a real go-getter who is excited about our vision for increasing the range of services we offer. 

This will include the responsibility for coordinating and managing existing groups such as SMART Recovery, and ensuring facilitation adheres to the models of delivery. As a member of the Assessments and Continuing Care team you may also undertake pre-treatment assessments and admissions as required.

Ideally, the successful candidate will have:

  • Relevant Health qualification in Alcohol and other drugs (AOD), social work or mental health
  • Be registered with a professional body or be willing and eligible to be registered.
  • Strong group facilitation skills as well as individual interventions
  • Strong relationship management and networking skills 
  • Motivational interviewing experience
  • Experience working in the AOD/mental health field
  • Excellent clinical judgement and strong clinical boundaries
  • An understanding of tikanga Maori and its importance within this sector
  • Report writing skills and computer proficiency
  • A strong ability to complete informative and up to date records

  • $65,915.00 - $73,590.00 per annum (depending on your relevant registration and experience)

As well as personal group delivery, you will maintain relationships with other facilitators and seek new opportunities for partnerships. Your direct engagement with clients leaving treatment will ensure they are linked with appropriate services within the hub and elsewhere to continue their recovery journey.
