Clinical Coordinator
We are looking for a skilled and compassionate Registered Nurse with strong community experience to support our diverse range of clients across Auckland, with a primary focus on elderly people living independently in the community. We are a not-for-profit organization providing Home and Community-based Nursing Services across Northland, Greater Auckland, and Southland/Otago/Canterbury regions.

Essential:
- NZ Registered Nurse with a current Annual Practising Certificate (APC).
- Local New Zealand clinical experience (community, primary care, rehabilitation, ACC pathways, or related).
- Strong clinical assessment skills and high attention to detail.
- Excellent written and verbal communication skills.
- Full New Zealand Driver Licence.
- Valid New Zealand work visa, residency, or citizenship (must be eligible to work in NZ).
What will you do?
No two days are the same for a Clinical Coordinator - this position will keep you engaged by:
- Managing a caseload of clients
- Assessing clients using InterRAI assessment tools
- Identifying support and rehabilitation needs
- Developing individualised support plans to ensure the needs of the people we support, and their whanau are met with care
- Providing clinical oversight to our support workers
- Building relationships, identifying opportunities, collaborating with other healthcare providers and thinking outside of the box!
